What Is Cagrilintide?
Cagrilintide is an experimental medicine being studied for weight management. It is designed to act like a natural hormone called amylin.
Your pancreas releases amylin when you eat. This hormone helps your body manage what happens after a meal, including signals involved in feeling full.
Cagrilintide is a modified, longer-lasting version of this type of hormone. Researchers developed it so its effects last much longer than natural amylin, allowing it to be studied as a once-weekly treatment. [6, 2, 13]
You may also see cagrilintide discussed alongside semaglutide, the active ingredient in medicines such as Wegovy. The combination of cagrilintide and semaglutide is called CagriSema.
Cagrilintide and CagriSema are different: cagrilintide is one compound; CagriSema contains two. Their weight-loss results should be described separately. [12]
How Does Cagrilintide Work?
The simplest explanation is that cagrilintide acts on signals involved in hunger and fullness.
These signals help the body decide when it has had enough food. Cagrilintide activates receptors—small receiving points on cells—that respond to amylin-like signals.
Laboratory studies show that it acts on both amylin and calcitonin receptors. Animal research also points to specific brain pathways involved in its effects on food intake and body weight. [7, 8, 9]
For a beginner, the key idea is:
Stronger fullness signals may help a person eat less, which can support weight loss over time.
Natural amylin also slows the movement of food out of the stomach. However, its general actions should not be used to assume that every effect has been measured to the same extent with cagrilintide. [13, 7]
What Are the Potential Benefits of Cagrilintide?
1. Weight Loss
Weight reduction is the clearest potential benefit supported by human studies.
In a 26-week clinical trial, cagrilintide produced greater average weight loss than placebo. A placebo is an inactive treatment used to help researchers measure whether a medicine makes a difference.
Under the analysis that assumed participants continued their assigned treatment, average weight loss ranged from approximately 6.0% to 10.8% across the cagrilintide groups, compared with 3.0% with placebo.
The results varied depending on the amount studied. These were trial averages, so they do not predict what any individual would lose. [1]
2. Support for Appetite Control
Cagrilintide’s action on amylin receptors provides a scientific reason to study it for appetite control.
Earlier human research with pramlintide, a different amylin-based medicine, found reduced food intake and smaller meals. These studies help explain the interest in amylin-based treatments, but their exact results cannot be assigned to cagrilintide. [7, 11, 10]
A careful way to describe this potential benefit is:
Cagrilintide acts on appetite pathways that may make eating less easier.
Claims that it completely removes hunger, stops cravings or eliminates “food noise” go beyond what the sources discussed here establish.
3. Greater Weight Loss When Combined With Semaglutide
Researchers are also studying whether cagrilintide can add to the effects of semaglutide.
The two compounds act on different hormone systems:
- Cagrilintide acts on amylin and calcitonin receptors.
- Semaglutide acts on GLP-1 receptors.
In the REDEFINE 1 trial, the combination produced greater average weight loss than either compound alone. This supports the potential benefit of targeting complementary appetite pathways. [7, 4, 12]
How Much Weight Loss Has Cagrilintide Shown?
Two studies are particularly helpful for understanding the results.
The Earlier Cagrilintide Study
The 2021 phase 2 trial included 706 adults with overweight or obesity who did not have diabetes.
Over 26 weeks, cagrilintide produced average weight reductions of approximately 6.0%–10.8%, compared with 3.0% with placebo, under the analysis assuming treatment adherence.
This study provided evidence that cagrilintide can reduce body weight on its own. [1]
The Larger REDEFINE 1 Study
REDEFINE 1 studied adults with overweight or obesity without diabetes for 68 weeks.
The following averages came from the analysis that accounted for treatment discontinuation and the use of other weight-management treatments:
| Study treatment | Average body-weight reduction |
|---|---|
| Cagrilintide alone | 11.5% |
| Semaglutide alone | 14.9% |
| CagriSema: both compounds together | 20.4% |
| Placebo | 3.0% |
Participants also received lifestyle support. These results describe the study population and conditions, rather than guaranteed outcomes. [4, 12]
Why Do Some Articles Report Around 23% Weight Loss?
You may see 20.4% and 22.7%, sometimes rounded to 23%, reported for CagriSema in the same trial.
They come from different ways of analysing the results:
- 20.4% accounts for treatment discontinuation and other weight-management treatment.
- 22.7% estimates what would happen if participants stayed on their assigned treatment without other weight-loss therapies.
Both numbers refer to CagriSema, rather than cagrilintide alone. The analysis used should always be stated alongside the result. [12]
Cagrilintide vs Semaglutide: What Is the Difference?
Both are being discussed in weight-management research, but they act on different targets.
| Question | Cagrilintide | Semaglutide |
|---|---|---|
| Which hormone system does it mimic? | Amylin-related signalling | GLP-1 signalling |
| Which receptors does it activate? | Amylin and calcitonin receptors | GLP-1 receptors |
| Can it support weight loss? | Human trials have shown weight reduction | Human trials have shown weight reduction |
| What is the combination called? | CagriSema when combined with semaglutide | CagriSema when combined with cagrilintide |
In REDEFINE 1, semaglutide alone produced greater average weight loss than cagrilintide alone, while the combination produced the largest average reduction. [7, 14, 12]
What Have Studies Found in People With Type 2 Diabetes?
CagriSema has also been studied in adults with type 2 diabetes.
In the 68-week REDEFINE 2 trial, average body-weight reduction was:
- 13.7% with CagriSema
- 3.4% with placebo
These findings concern the combination treatment. They should not be described as results for cagrilintide alone. [5]
An earlier phase 2 trial also compared the combination with its individual components and examined blood-sugar control. This research helps scientists understand how the treatments may work in people with both diabetes and excess weight. [3]
What Are the Possible Side Effects of Cagrilintide?
In the cagrilintide-alone phase 2 trial, the most frequent side effects included:
- Nausea
- Constipation
- Diarrhoea
- Reactions around the injection site
Digestive side effects occurred more often in the cagrilintide groups than in the placebo group. Their frequency varied across the amounts studied. [1]
Early combination research also reported digestive symptoms, with most reported adverse events being mild or moderate. However, a treatment’s safety cannot be judged only by whether its common side effects are manageable. Larger and longer studies help identify less common problems. [2]
Effective weight loss does not mean a treatment is suitable for everyone.
What Do We Still Need to Learn?
The research provides meaningful evidence for weight reduction, but it does not answer every question.
When assessing cagrilintide, it is useful to distinguish:
- Weight loss during a trial from weight maintenance over many years.
- Common side effects from rare or delayed safety problems.
- Cagrilintide-alone findings from combination-treatment findings.
- Changes in body weight from proven reductions in heart attacks, strokes or other long-term complications.
The trials discussed here support weight-loss claims within their study conditions. Those findings should not be stretched into promises of lifelong results or broad disease prevention. [1, 4, 5]
Is Cagrilintide Approved for Weight Loss?
Cagrilintide has been investigated as a standalone treatment and as part of CagriSema.
Novo Nordisk announced a US approval application for CagriSema in December 2025. Its 20 October 2026 update stated that the combination was still unapproved in the United States at that time.
An application is different from an approval, and approval status must be checked for the specific product and country. [14, 15]
Clinical trial findings relate to the medicines used in those trials. They do not establish that products sold online as research peptides have the same quality, safety or effectiveness.
Frequently Asked Questions About Cagrilintide
What is cagrilintide used for?
Cagrilintide is being studied for weight management. Researchers have investigated it alone and alongside semaglutide, including in people with type 2 diabetes. [1, 3]
Is cagrilintide a GLP-1 medicine?
No. Cagrilintide acts on amylin and calcitonin receptors. Semaglutide, the other component of CagriSema, acts on GLP-1 receptors. [7, 14]
Does cagrilintide help with weight loss on its own?
Yes, clinical trials have shown weight reduction with cagrilintide alone. In REDEFINE 1, average weight loss was 11.5% over 68 weeks, compared with 3.0% with placebo, under the analysis accounting for treatment discontinuation and other weight-management treatment. [12]
Is cagrilintide the same as CagriSema?
No. CagriSema combines cagrilintide and semaglutide. Cagrilintide is only one of its two components. [14]
Is cagrilintide better than semaglutide?
It depends on the outcome being compared. For average weight loss in REDEFINE 1, semaglutide alone outperformed cagrilintide alone. The combination outperformed both individual treatments. That comparison does not determine which treatment would be suitable for an individual. [12]
Does cagrilintide burn fat?
The clearest human evidence discussed here is for body-weight reduction. Describing it simply as a “fat burner” can mislead readers: its studied mechanism involves appetite-related signalling, and body-weight loss is not automatically the same as pure fat loss. [1, 7]
How quickly does cagrilintide work?
The main results discussed here were measured over 26 weeks and 68 weeks. They do not establish a guaranteed time for someone to notice changes in hunger or weight. [1, 4]
What are the most common cagrilintide side effects?
The cagrilintide-alone phase 2 trial commonly reported nausea, constipation, diarrhoea and injection-site reactions. [1]
Can cagrilintide be combined with semaglutide?
The combination has been studied in clinical trials under the name CagriSema. That evidence concerns supervised treatment with defined trial medicines; it is not an instruction to combine products independently. [2, 4]
Does everyone lose the same amount of weight?
No. Study results are averages. Individual responses, side effects and the ability to continue treatment can differ. The different analyses reported in REDEFINE 1 also show why treatment continuation matters when interpreting results. [12]
